EMS Annual Awards Ceremony 2016
The Utah Department of Health, Bureau of Emergency Medical Services and Preparedness, will be hosting the Awards Ceremony on Wednesday, July 13, 2016, at 10:00 a.m. – noon.
The nominations forms for Individual Outstanding Performance and the Incident of the Year are available below. The nomination deadline is June 1, 2016 and the incident dates should be between April 1, 2015 through April 1, 2016.
