Course Coordinators

The spreadsheet below is provided at your benefit to assist with obtaining psychomotor skills examiners. The Bureau does not endorse any individual on the list. It is the responsibly of the course coordinator to utilize the public portal search to verify credentials of EMS Instructors. To obtain the password to open the spreadsheet please email ems@utah.gov.

INITIAL EDUCATOR CERTIFICATION

Dates and locations for future certification training courses can be found in Upcoming Events. Courses for initial certification will be offered on the following dates and locations in 2018:

  • New Instructor
  • January 3-5 – Sandy
  • July 25-27 – Hurricane
  • September 26-28 – Lehi
  • New Instructor for Current Educators
  • January 24 – SLC
  • February 22 – St. George
  • August 15 – Ogden
  • New Training Officer
  • February 22 – St. George
  • July 27 – Hurricane
  • October 2 – SLC
  • New Course Coordinator
  • July 27 – Hurricane
  • October 2 – SLC

 

EMS Instructor Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA or Paramedic.
  2. Submit three (3) letters of recommendation outlining and describing your EMS skills and teaching abilities.
  3. Submit documentation to verify you have fifteen (15) hours of instruction experience within the last 12 months.
  4. Submit documentation to verify that you have provided thirty (30) hours of actual patient care within the past year prior to certification (EMT, AEMT, EMT-IA, or Paramedic only).
  5. Complete an EMS Instructor course and sign a contract.
  6. Pay the $150 certification fee.

EMS Instructor for Current Educator Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA, or Paramedic.
  2. Must be a currently certified Fire Instructor I or Fire Instructor II (you must submit a copy of your Fire Instructor certificate) or previous EMS Instructor.
  3. Submit three (3) letters of recommendation outlining and describing your EMS skills and teaching abilities.
  4. Submit documentation to verify you have fifteen (15) hours of instruction experience within the past 12 months.
  5. Submit documentation to verify that you have provided thirty (30) hours of actual patient care within the past year prior to certification (EMT, AEMT, EMT-IA, or Paramedic only).
  6. Complete an EMS/Fire Instructor course and sign a contract.
  7. Pay the $150 certification fee.

EMS Course Coordinator Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA, or Paramedic.
  2. Must be a currently certified EMS Instructor.
  3. Must be the co-coordinator-of-record for a class coordinated by a current Course Coordinator.
  4. Submit a letter of recommendation from the Course Coordinator including the course number.
  5. Complete an EMS Course Coordinator course and sign a contract.
  6. Pay the $75 certification fee.

EMS Training Officer Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA, or Paramedic.
  2. Must be a currently certified EMS Instructor.
  3. Complete an EMS Training Officer course and sign a contract.
  4. Pay the $75 certification fee.

 

RENEW EDUCATOR CERTIFICATION

Dates and locations for future certification training seminars can be found in Upcoming Events. Seminars for all renewals will be offered on the following dates and locations in 2018:

  • February 23 – St. George
  • May 18 – Brigham City
  • August 17 – Sandy

 

EMS Instructor Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA, or Paramedic.
  2. Must be a currently certified EMS Instructor.
  3. Complete renewal course for EMS Instructor.

EMS Course Coordinator Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA, or Paramedic.
  2. Must be a currently certified EMS Instructor.
  3. Must be a currently certified EMS Course Coordinator.
  4. Complete renewal course for EMS Course Coordinator.
  5. Submit EMS Course Coordinator contract.

Training Officer Certification

  1. Must be currently licensed in Utah as an EMD, EMR, EMT, AEMT, EMT-IA, or Paramedic.
  2. Must be a currently certified EMS Instructor.
  3. Must be a currently certified EMS Training Officer.
  4. Complete renewal course for EMS Training Officer.
  5. Submit EMS Training Officer contract.
