EMS Instructors

If you are interested in helping the EMS programs in Utah with practical skills evaluation pleases complete the form below. This information will be shared with all course coordinators in Utah. Please only provide contact information you are comfortable sharing. Completing this from does not guarantee any program will contact you. Course Coordinators

The Bureau has available a list for your benefit to assist with obtaining psychomotor skills examiners. The Bureau does not endorse any individual on the list. It is the responsibly of the course coordinator to utilize the public portal search to verify credentials of EMS Instructors. To obtain a copy of the spreadsheet please email ems@utah.gov.

1. Must be currently licensed in Utah as an EMR, EMT, AEMT, or Paramedic.
2. Submit three (3) letters of recommendation outlining and describing your EMS skills and teaching abilities.
3. Submit documentation to verify you have fifteen (15) hours of instruction experience within the last 12 months.
4. Submit documentation to verify that you have provided thirty (30) hours of actual patient care within the past year prior to endorsement (EMT, AEMT, or Paramedic only).
5. Complete an EMS Instructor course and sign a contract.
6. Pay the $150 endorsement fee.

1. Must be currently licensed in Utah as an EMR, EMT, AEMT, or Paramedic.
2. Must be a currently certified EMS Instructor.
3. Must be the co-coordinator-of-record for a class coordinated by a current Course Coordinator.
4. Submit a letter of recommendation from the Course Coordinator including the course number.
5. Complete an EMS Course Coordinator course and sign a contract.
6. Pay the $75 endorsement fee.

2. Must be a currently certified EMS Instructor.
3. Complete an EMS Training Officer course and sign a contract.
4. Pay the $75 endorsement fee.

3. Complete renewal course for EMS Instructor.

1. Must be currently licensed in Utah as an EMR, EMT, AEMT,  or Paramedic.
2. Must be a currently endorsed EMS Instructor.
3. Must be a currently endorsed EMS Course Coordinator.
4. Complete renewal course for EMS Course Coordinator.
5. Submit EMS Course Coordinator contract.

2. Must be a currently endorsed EMS Instructor.
3. Must be a currently endorsed EMS Training Officer.
4. Complete renewal course for EMS Training Officer.
5. Submit EMS Training Officer contract.
