The Office of Emergency Medical Services and Preparedness licenses and regulates the following EMS levels:
- Emergency Medical Responder
- Emergency Medical Technician
- Advanced Emergency Medical Technician
- Paramedic
- Emergency Medical Dispatcher

INITIAL LICENSURE
- Successfully complete state-approved course – Available training courses can be found at https://emslicense.utah.gov/lookup/training_courses
- Complete application at https://emslicense.utah.gov within 30 days course start date.
- Pay fees.
- Submit DACS fingerprints for criminal background check (one time requirement).
- Successfully complete the state-approved National Registry cognitive and psychomotor exams.

RENEW LICENSE
- Complete application at https://emslicense.utah.gov
- Must submit a passport type photo.
- Must submit a government identification.
- Must verify completion of BLS Provider CPR Course (all levels).
- Must verify completion of PEPP, PALS, EPC or equivalent for AEMT and Paramedic Applications.
- Must verify ACLS for Paramedic Applications.
- Pay fees.
- Submit DACS fingerprints for criminal background check (one time requirement).
- Complete CME Verification on NREMT.org by March 31st for EMT, AEMT, Paramedics and September 30th for EMR. EMD applications do not have this requirement and are only required to verify proof of EMD training.

RECIPROCITY
- Complete application at https://emslicense.utah.gov
- Pay fees.
- Submit DACS fingerprints for criminal background check (one time requirement).
- Submit current EMS license from another state and/or NREMT.
- If not NREMT certified, successfully complete the NREMT examination process.
ADDITIONAL INFORMATION
State Approved Refresher Course -In order to satisfy the requirement for a Utah approved refresher an individual is required to follow the NCCP National Component Each level for EMR, EMT, AEMT, and Paramedic has different hour requirements as well as topic areas. Below is the 2016 NCCP National Component requirements.
NCCP National Component Requirements
Fingerprints – Fingerprint submission is a one-time requirement. After your fees and application have been processed by the Office, you will receive email notification that your LiveScan fingerprint form is available to download from our system. You will take the LiveScan form to any Direct Access Clearance System (DACS) LiveScan location for electronic submission of fingerprints. The fingerprint record will be maintained by the Office for ongoing background check monitoring as required for Utah licensed EMS personnel. Questions please email: emsdacs@utah.gov
Criminal Background Check Information
Written and Practical Exams – Your EMS Course Coordinator will arrange and schedule your NREMT practical exam as part of your course. You will need to create a profile on http://www.nremt.org/ before completing your practical test so the certification office can update the requirements for you to schedule the test. The written test is taken through National Registry at a Pearson Vue testing center. The Office of Emergency Medical Services and Preparedness will verify your NREMT certification status.
Purchasing Patches or Decals
EMS personnel may purchase patches or decals from the Office of Emergency Medical Services and Preparedness. Only currently licensed EMS personnel may purchase these items, and they may only purchase items that match their current level of licensure.
