The Bureau of Emergency Medical Services and Preparedness licenses and regulates the following EMS levels:

  • Emergency Medical Responder
  • Emergency Medical Technician
  • Advanced Emergency Medical Technician
  • Paramedic
  • Emergency Medical Dispatcher

EDUCATION RESOURCES

 

INITIAL LICENSURE

Emergency Medical Responder and Emergency Medical Technician

  1. Successfully complete state-approved course – Available training courses can be found at https://emslicense.utah.gov/lookup/training_courses
  2. Complete application at https://emslicense.utah.gov
  3. Pay fees.
  4. Submit results of tuberculosis test .
  5. Submit DACS fingerprints for criminal background check (one time requirement).
  6. Submit CPR Provider Card – CPR class must meet the AHA guidelines for Health Care Provider.
  7. Successfully complete the state-approved National Registry cognitive and psychomotor exams.

Advanced Emergency Medical Technician

Prerequisite: Applicant must hold current EMT license.

  3. Pay fees.
  4. Submit results of tuberculosis test.
  5. Submit DACS fingerprints for criminal background check (one time requirement)

  7. Successfully complete the state-approved National Registry cognitive and psychomotor exams.

Paramedic

Prerequisite: Applicant must hold current EMT or AEMT license.

  4. Submit results of tuberculosis test.

  7. Submit Advanced Cardiac Life Support card.
  8. Submit Pediatric Education for Prehospital Professionals (PEPP) or Pediatric Advanced Life Support (PALS).
  9. Successfully complete the state-approved National Registry cognitive and psychomotor exams.

Emergency Medical Dispatcher

  1. Successfully complete state-approved course  OR   Submit current International Academies of Emergency Dispatch certification.   Available training courses can be found at https://emslicense.utah.gov/lookup/training_courses
  2. Complete application at https://emslicense.utah.gov
  3. Pay fees.
  4. Submit DACS fingerprints for criminal background check (one time requirement).

Emergency Medical Responder

  3. Submit DACS fingerprints for criminal background check (one time requirement).
  4. Submit results of tuberculosis test.
  5. Submit CPR Provider Card – CPR class must meet the AHA guidelines for Health Care Provider.
  6. Submit 52 hours CME as outlined in the Recertification Protocols for Emergency Medical Services Personnel.
  7. Submit verification of demonstration of the psychomotor skills listed in the current National EMS Education Standards for this licensure level.

Emergency Medical Technician

  3. Submit DACS fingerprints for criminal background check (one time requirement).
  4. Submit results of tuberculosis test.
  5. Submit CPR Provider Card – CPR class must meet the AHA guidelines for Health Care Provider.
  6. Submit 98 hours CME as outlined in the Recertification Protocols for Emergency Medical Services Personnel.
  7. Submit verification of demonstration of the psychomotor skills listed in the current National EMS Education Standards for this licensure level.

Advanced Emergency Medical Technician

  3. Submit DACS fingerprints for criminal background check (one time requirement).
  4. Submit results of tuberculosis test.
  5. Submit CPR Provider Card – CPR class must meet the AHA guidelines for Health Care Provider.
  6. Submit Pediatric Education for Prehospital Professionals (PEPP), Pediatric Emergency Assessment, Recognition and Stabilization (PEARS), or equivalent.
  7. Submit 108 hours CME as outlined in the Recertification Protocols for Emergency Medical Services Personnel.
  8. Submit recommendation and verification of demonstrated proficiency in the psychomotor skills (listed in the current National EMS Education Standards) from a certified off-line medical director.

Paramedic

  2. Pay fees.
  3. Submit DACS fingerprints for criminal background check (one time requirement).
  4. Submit results of tuberculosis test.
  5. Submit CPR Provider card – CPR class must meet the AHA guidelines for Health Care Provider.
  6. Submit Advanced Cardiac Life Support card.
  7. Submit Pediatric Education for Prehospital Professionals (PEPP) or Pediatric Advanced Life Support (PALS).
  8. Submit 144 hours CME as outlined in the Recertification Protocols for Emergency Medical Services Personnel.
  9. Submit recommendation and verification of demonstrated proficiency in the psychomotor skills (listed in the current National EMS Education Standards) from a certified off-line medical director.

Emergency Medical Dispatcher

    3. Submit DACS fingerprints for criminal background check (one time requirement).
    4. Submit CPR Provider Card – CPR class must meet the AHA guidelines for Health Care Provider.
    5.  Submit 48 hours CME as outlined in the Recertification Protocols for Emergency Medical Services Personnel  OR  Submit proof of current IAED certification and completion of a 2 hour CISM course.

 

RECIPROCITY

EMR, EMT, AEMT, and Paramedic

  3. Submit DACS fingerprints for criminal background check (one time requirement).
  4. Submit current EMS license from another state or NREMT.
  5. Submit CPR Provider card – CPR class must meet the AHA guidelines for Health Care Provider.
  6. Submit Pediatric Education for Prehospital Professionals (PEPP) or Pediatric Advanced Life Support (PALS) (Advanced EMT and Paramedic only).
  7. Submit Advanced Cardiac Life Support card (Paramedic only).
  8. Submit 25 hours of CME completed within one year of application  OR  Submit proof of initial certification/licensure completed within one year of application.
  9. If not NREMT certified, successfully complete the NREMT assessment exam corresponding to the level of license being sought.

 

ADDITIONAL INFORMATION

Fingerprints – Fingerprint submission is a one-time requirement. After your fees and application have been processed by the Bureau, you will receive email notification that your LiveScan fingerprint form is available to download from our system. You will take the LiveScan form to any Direct Access Clearance System (DACS) LiveScan location for electronic submission of fingerprints. The fingerprint record will be maintained by the Bureau for ongoing background check monitoring as required for Utah licensed EMS personnel.

Criminal Background Check Information

Written and Practical Exams – Your EMS Course Coordinator will arrange and schedule your NREMT practical exam as part of your course. You will need to create a profile on http://www.nremt.org/ before completing your practical test so the certification office can update the requirements for you to schedule the test. The written test is taken through National Registry at a Pearson Vue testing center. The Bureau of EMS will verify your NREMT certification status.

Purchasing Patches, Decals, and Replacement License Cards
EMS personnel may purchase patches, decals, and replacement cards from the Bureau of EMS. Only currently licensed EMS personnel may purchase these items, and they may only purchase items that match their current level of licensure.

To order patches, decals, and replacement certification cards, please visit or contact the Bureau of EMS. For replacement certification cards, we recommend sending a check, along with a self-addressed stamped envelope and a current photo of yourself, but we also take orders by phone, by e-mail, and in person.
